Since 1997, VCH has provided more than $4 million dollars each year in grants to support community partners in their work to help empower vulnerable communities to increase control over, and improve, their health through addressing upstream social and/or environmental barriers. SMART's community health promotion model builds on the long-term relationships that community organizations have with the populations the fund intends to serve. Few are better able to understand the unique needs of youth, mental health consumers, seniors or substance users than those who are part of the communities themselves. SMART long-standing support of peer-led health promotion initiatives have had lasting impact in the lives of thousands of residents in the VCH region. SMART's investments leverage support from other funders as well as volunteers augmenting the impact of the health authority dollars.
